How to access the Cardio Care Programme

You must consult with a nominated Premier Plus GP to access the Cardio Care Programme, as well as be registered for at least one of the following conditions as part of the Chronic Illness Benefit (CIB):

  • Hypertension
  • Ischaemic heart disease
  • Hyperlipidaemia.
